Forms to record changes to personal
details are available from
reception.
If you move outwith the practice boundary you are advised to register with a GP near to your new locality. Once you have signed up your records will be recalled and automatically forwarded to your new surgery.
Please note
that if you are waiting for an
appointment from a local hospital,
then you must inform the hospital of
your change of address.
